Cold starting/ Rough running

Ive noticed my motor is running rough on cold starting. Its a CD20 engine with 156k on the clock. When starting from cold, I hear no real engine turning, but it starts ok. After this it runs rough for a few secs with RPM fluctuating between 500 and 1200 rpm, sounding like its going to cut out and emitting smoke from the exhaust when increasing throttle. It clears itself within around 5 secs and runs prefectly thereafter and only happens on cold mornings. Any thoughts?
 
last serviced 1000 miles ago, all filters were changed and in addition, I use a very good injector cleaner. The injectors were overhauled 3000 miles ago.
 
well after much searching on t'internet, a common way of cold starting diesels is appearently to leave the ignition on (just the bit that operates the radio) for 15 secs, then try starting. So today, after her sitting for 2 days in sub zero temperatures I thought id give it a go and wahey, perfect engine start with no rough running or smoke emission. Ill have to give it a few more goes to elimninate the chance it could be a coincidence though.
 
i had to do this sometimes with my old 306 dt. ignition on, glow plug light on, ignition off. then same agin, then it would start alryt keeping the revs @ 1500rpm for 2 secs.
