how get off the gear knob? pull or turn?

turn bud, anti clockwise - will be hard to do if its never been off.

Get a rag, a mole grip, put the rag around the gear knob then put the mole grip on tight, turn it hard. Should come off with little damage.

I used a hacksaw on mine, also cut the shifter about an 1" at the same time. Ghetto shortshift FTW.
